There are some people who fear to get the covid-19 during our life we want to protect ourselves among our family member. I will give you a guide I went through and in order before you can get your actual vaccine there will be paper work to do.
Step 1:
Download the eZ Consult on your phone by going to the Play Store.
Step 2: Click Sign Up
Click Sign Up as Patient
You will receive a text message to type in before you can proceed to enter all your information.
Make sure your name is on your identification that you will be giving and make sure you bring it with you when you're vaccine is schedule.
Step 3:
Click FOR MYSELF
Tip:
Make sure you take a picture of your identification with picture to upload it from your phone to add to the eZ Consult.
Click VACCINATION
Select Type of vaccination you want for yourself than click next
Fill up your address click ACCOUNT located right hand side in the bottom
Click add on the right hand side.
Tip:
Make sure you have a copy of your prescriptions and bring it when you have your vaccine no prescription no vaccine for you.
You will receive an email once it has been approved by the reviewer it only take a short time until you will receive instructions where you need to get your vaccination.
Approved you will fill out the following information on the eZ Consult
Make sure you bring a pen you will be filling up additional documents when you reach the area of your vaccination with a screen shot or printed of your QR Code.
Final Tips:
- Bring the following with you a letter from your physician allowing you to take the vaccine, prescription for your medical history, identification with full name used for when you had signed in the eZ Consult and a pen.
- Make sure you wear a facial mask and face shield.
- Best to wear short sleeve shirt or sleeveless to get your vaccine.
- Proceed to the area where you get your vaccine and have your documents available to present to the person at the area.
- You will do registration in the vaccine facility and be instructed where you will go.
- Someone will also take your temperature and blood pulse.
- Bring alcohol with you.
- After your vaccine you will be instructed to be seated for 15 minutes before you can go home to make sure you have no reaction.
- You will notify when your next vaccine will be taken for your next visit be sure to bring your Vaccination Card with you and Identification with you.
